Completing forms is never fun but the information they provide is invaluable to the practice and to the doctor. For this reason, you are encouraged to complete forms prior to coming in for your appointment.
Please also come prepared to present a valid photo I.D. and insurance card to the receptionist. Payments of co-pays and/or cash pays are expected at the time of service or a $20 administrative fee may apply.
Also, please bring a list of the medications you take for your medical record. If you have any X-rays or diagnostic films it is highly recommended that you bring them with you to your appointment. |
